A Failure Mode and Effect Analysis FMEA is a systematic proactive method for identifying and preventing product and process problems before they occur. FMEAs are focused on preventing defects, enhancing safety and increasing customer satisfaction. FMEAs are conducted in the product design or process development stages, although conducting an FMEA on existing products and processes can also yield substantial benefits. Six Sigma's project team use FMEA in the Analyze stage of DMAIC because extraordinary quality is not only designed into the product, but also into the development process itself.

FMEA is a proactive risk management tool designed to answer three critical questions: What could go wrong? What are the consequences? How can we prevent it? It serves as a bridge between theory and execution, helping teams quantify risks and build a culture of prevention rather than reaction. Within the Six Sigma DMAIC framework, it is an essential part of the Analyze phase, driving data-based decisions to design quality directly into the system.

By evaluating the severity, occurrence, and detectability of risks, teams can prioritize and mitigate issues early in the development cycle.
